Certified Nurse Assistant CNA

Functions as a para-professional member of the Hospice team supervised by a Patient Care Manager. Perform various services for a patient as necessary to meet the patient's personal needs, to promote the patient's comfort, and a safe environment. Observe patients and report/document these observations and the services provided. Manage related expenditures in a fiscally responsible manner in accordance with the Company's budget. Travel as necessary.


Minimum age requirement of 18.
Completion of recognized Nurse Assistant certification program
Passing score on Transitions Hospice skills test.
Minimum of one-year direct patient care experience in an institutional or home care setting required. Hospice experience preferred.
Skilled in the proper technique for lifting and transferring patients.
Ability to read, write and follow instructions and document care given.
Understands Hospice philosophy and needs of terminally ill.
Compliance with accepted professional standards and practices.
Demonstrates tact, patience and good personal hygiene.
Excellent observation and communication skills.
Ability to work within an interdisciplinary setting.
Physical exam in accordance with state law requirements.
Valid driver's license with maintenance of a good driving record and reliable transportation that is insured in accordance with state and/or Transitions requirements and is in good working order.
Satisfactory references from employers and/or professional peers.
Satisfactory criminal background check.
Self-directed with the ability to work with little supervision.
Flexible and cooperative in fulfilling all obligations.


Respond to patient's/family's/caregiver's needs promptly. If unable to perform a certain task, report to the Patient Care Manager immediately.
Meet safety needs of patients and use equipment safely and properly (foot stools, side rails, restraints, etc.)
Report on patient's condition and significant changes to the Case Manager/Patient Care Coordinator.
Keep patient's living area clean and orderly, as appropriate.

Ensure quality and safe delivery of home Hospice services to patients/families/caregivers.
Perform or assist patient with personal care activities as prescribed in the care plan, including but not limited to hygiene, mobility and elimination. If unable to perform a certain task, report to Case Manager/Patient Care Manager immediately.
Practice universal precautions in all contacts with patients.
Perform or assist patient/family with home maintenance activities as prescribed in the care plan including but not limited to supporting independence, providing companionship, and providing respite for families.
Respect the autonomy and right to confidentiality of all patients and families.
Monitor adequate fluid and nutrition intake, temperature, pulse, respiration, skin and basic elements of body functioning and changes in body functioning and record/report appropriately.
Practice safe transfer, ambulation, normal range of motion and positioning techniques.
Identify need for intervention by other members of the interdisciplinary team on an ongoing basis and report to primary case manager.
Provide input into interdisciplinary evaluation of effectiveness of care.
Document care provided and response of patient/family. Submit documentation in a timely manner.
Manage a caseload and deliver care in a cost-effective manner that demonstrates an understanding of hospice reimbursement methodologies.
Participate in evening/weekend on-call or overtime as required, conducting on-call services in a competent and responsive manner.
Work in cooperation with family/caregiver/and interdisciplinary team members to meet the emotional needs of the patient/family/caregiver.
Assure for compliance with local, state and federal laws, Medicare regulations, and established personnel policies and procedures.
Participate in and attend team meetings, staff meetings department meetings, briefings, in-services, committees and other related activities as needed. Contribute positively to patient care planning.
Complete a minimum of 12 hours of in-services per year. Attend memorial or funeral services quarterly.
Use supplies and equipment effectively and efficiently.
Regular attendance is to be maintained.
Adherence to a code of conduct conducive with Transitions HealthCare policy is expected.
Meet or exceed delivery of Company Service Standards in a consistent fashion.
Interact with all staff in a positive and motivational fashion supporting the Company's mission.
Conduct all business activities in a professional and ethical manner.
Create and maintain a professional atmosphere that is also a fun place to work.

General offices, extended care facility, patient home setting, and exposure to infectious diseases, travel by automobile and occasionally by airplane.


Requires physical exertion. Duties of patient care during transfer activities (adjusting the patient in bed, transferring the patient from bed to wheelchair or vice versa, and chair to toilet, etc.) require that the employee have the physical ability to maneuver patients according to the care required. Additional physical abilities required during these activities will vary according to the level of patient cooperativeness, patient weight, available space and available equipment. Regularly required to use hands, talk or hear. Specific vision abilities include close, peripheral, depth perception, and ability to adjust focus. Must be able to travel to businesses, offices, or patient homes or where conditions may be adverse due to weather, environment, or patients' lifestyles.

Don't Be Fooled

The fraudster will send a check to the victim who has accepted a job. The check can be for multiple reasons such as signing bonus, supplies, etc. The victim will be instructed to deposit the check and use the money for any of these reasons and then instructed to send the remaining funds to the fraudster. The check will bounce and the victim is left responsible.
